When Your Teen Is Struggling, It’s Hard to Know What Level of Help They Need

If you’re a parent or guardian watching your teen struggle with anxiety, depression, school refusal, substance use, or emotional outbursts, you may feel stuck in the same painful question:

What Is a PHP Program for Teens?

A Day Treatment program, also known as a Partial Hospitalization Program (PHP), is a structured, intensive treatment program designed for teens who need more support than weekly outpatient therapy but do not require inpatient hospitalization.

What Is an IOP Program for Teens?

An Intensive Outpatient Program (IOP) is a step down from PHP, meaning less intensive than PHP. It provides strong clinical support while allowing teens to maintain more of their normal routine.

New Harbor’s Teen IOP treatment includes:

  • Group therapy several days per week
  • Weekly individual therapy
  • Family involvement and support
  • Structured treatment planning
  • Coping skill development

IOP is often ideal for teens who need meaningful support but are stable enough to manage part-time treatment.

Important Note for parents and guardians still reading! New Harbor operates one of the few in-person after-school IOP programs in Massachusetts that is open 5 days a week. This allows teens to continue with treatment while attending school and even extra-curricular activities. 

PHP vs IOP: What’s the Difference?

Here’s the simplest way to understand it:

PHP = Treatment is the main focus of the day

  • More hours per week
  • More structure
  • Higher support and monitoring
  • Teens will eat lunch at the program and receive an hour of academic support.

IOP = Treatment supports daily life

  • Fewer hours per week
  • Allows for school and activities!
  • Still clinically intensive

Many teens start in PHP and later transition into IOP.

Signs Your Teen May Need PHP or IOP

Every teen is different, but the most common signs include:

1. Emotional symptoms are escalating

  • Panic attacks
  • Intense mood swings
  • Frequent crying or withdrawal
  • Irritability and anger that feels “out of control”

2. School refusal or academic decline

If your teen can’t attend school regularly, or grades are dropping rapidly, it’s often a sign of deeper mental health distress.

3. Increased substance use

Warning signs include:

  • Vaping or marijuana use becoming daily
  • Alcohol misuse
  • Pills or unknown substances
  • Lying about use or stealing money

4. Self-harm or suicidal ideation

If your teen has expressed suicidal thoughts, has a history of self-harm, or you’ve noticed concerning behaviors, it’s essential to seek help immediately.

PHP or IOP may be appropriate depending on the level of safety risk.

5. Outpatient therapy isn’t enough

If your teen is already in therapy but symptoms continue to worsen, they may need a higher level of support.
